In order to expedite the insurance and payment process, please follow the below policies:

We recommend that you review your specific insurance plan for co-pay, deductible and other benefit information prior to your office visit. As there are hundreds of insurance plans, it is the patient’s responsibility to check with your insurance company to ensure we accept your individual plan. If we do not participate with your health insurance, you will be responsible for the full amount of the office visit and any related services.

Please bring your insurance card and prior authorization information (if necessary) with you to your appointment.

At the time of your office visit, you may be responsible for the following charges:

  • Insurance Co-Pays (due at time of check-in)
  • Annual Insurance Deductible
  • Coinsurance Amounts
  • Charges for any services and supplies not covered by insurance
  • Any services denied by insurance
  • Cosmetic Services (are not covered by insurance)

Any non-insurance visits (for either medical or cosmetic services) are due in full once services are rendered.

  • Personal Checks (NSF checks will be billed amount due plus $50 per occurrence)

Policies & Forms

If you are visiting Penrose Dermatology PLLC for the first time, please plan on arriving 15 minutes prior to your scheduled appointment. This will allow our team to gather the necessary information needed at the time of registration. Alternatively you can fill out the required forms online prior to your visit here.

All patients are required to fill out the following forms electronically:

  • General Demographics – under myInfo
  • Insurance Information – under myInfo
  • Medical History – under myVisits

Please bring identification, your insurance card and a list of all current prescription medications.

At Penrose Dermatology, we value our patient’s time, and strive to run on time. We intentionally do not overbook our schedule to ensure the best experience for your visit. If for any reason you need to cancel your appointment, please notify the office 24 hours in advance. In the event that you are unable to provide sufficient notice of a change in your appointment, you may be assessed a $25 cancellation fee.

Prescription Refills

In general, if you have run out of refills on your prescription(s), any refill requests should be made directly through your pharmacy. In some instances, certain medications require patients make periodic visits for refills. If it has been over 6 months since your last visit and you are looking to refill a prescription, please call the office as we require you to schedule a follow-up appointment.
